Patient Forms
New patient forms can be downloaded and completed prior, located in the New Patient Center tab, look for drop down On Line forms. This paperwork is also available in the clinic if you would prefer to complete at your first visit. On your first appointment be sure to bring the completed forms, your DL and Insurance cards.
Consultation
An initial consultation with Dr. Robert Facca occurs at your first visit, where you will discuss your chief complaint, other health-related problems and concerns. This initial visit is designed for Dr. Robert Facca to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
Examination
After your consultation, Dr. Robert Facca will perform a complete chiropractic / orthopedic / neurological exam.
X-Ray Studies
An X-ray is most often prescribed in order for Dr. Facca to see what any spinal degeneration and or pathology . Dr. Facca will review the x-ray and report of findings with each patient to gain an understanding of the condition of their spine.
Treatment
At the conclusion of this initial appointment, Dr. Facca will discuss a treatment plan to ensure the patient understands the commitment to gain results.
Lifestyle Improvements
The doctor will suggest a wellness program to incorporate outside of treatment. This may include activities or positions to avoid, at home exercises and/or stretches. The purpose of our wellness program is for you to be involved in taking charge of your health and achieve good spinal alignment.
